You are the anaesthetic registrar on call for the night shift. Over a short period you are bleeped about several tasks at once:

• A category 1 caesarean section
• An emergency laparoscopic appendicectomy
• A critically unwell patient in A&E Resus
• A post-operative patient on the ward whose pain is not controlled with their current analgesia
• Difficult vascular access on the surgical ward

Example answer
I would not simply respond to bleeps in the order they arrive. I would first clarify the true clinical urgency of each task, because the labels alone are not enough: the critically unwell patient may have an immediate airway problem, and the uncontrolled pain could signal a serious complication.
  • Get a brief, structured handover on each task and identify any immediate threats to life.
  • Recognise that the category 1 caesarean and the Resus patient may both need senior anaesthetic input at the same time.
  • Escalate early and mobilise help rather than trying to complete every task myself in sequence.
The core of a strong answer is coordinating limited resources and keeping oversight of several patients, not ranking cases in isolation.

How would you prioritise the tasks, and why?
Example answer
Priorities depend on clinical urgency, the location and availability of staff, and whether another clinician can safely manage a task. My reasoning would be:
  • Category 1 caesarean and the Resus patient: simultaneous, time-critical emergencies. Escalate immediately and allocate separate senior anaesthetic resources where possible.
  • Post-operative uncontrolled pain: assess promptly to exclude a serious complication, though this can often be delegated initially.
  • Emergency appendicectomy: delay if the patient is stable, ensuring monitoring, antibiotics, fluids and analgesia continue.
  • Difficult vascular access: lowest priority, unless access is needed for urgent treatment or resuscitation.
This is only one reasonable order. A clear, safe rationale matters more than the exact sequence.

Who else in the team could you delegate to?
Example answer
I would use the full range of staff across the hospital rather than carry the workload alone:
  • Another anaesthetist or the on-call consultant for one of the two time-critical emergencies.
  • The ward doctor or acute pain team to assess the post-operative patient.
  • Critical care outreach or a vascular access team for the difficult access.
  • The surgical team to keep monitoring and treating the patient awaiting appendicectomy.
Any delegation needs a clear handover, a colleague with the right skills, agreed escalation criteria and ongoing review. I would keep overall situational awareness and responsibility for coordinating the workload.

Is there anything else you would do?
Example answer
I would inform the on-call anaesthetic consultant of the competing priorities, so they know the on-call team is covering several urgent areas and may need to provide extra support to keep anaesthetic cover safe.

If I were also supporting the ICU SHO, I would inform the on-call ICU consultant, and advise the SHO to contact that consultant directly if they had concerns while I was unavailable, for example during the category 1 caesarean.

How we keep our Question Banks up to date

The Anaesthetics ST4 interview changes from year to year, and the biggest fear for any candidate is getting to interview day and realising you've revised with a question bank that's totally outdated.
