A 45-year-old man treated with packed red cell transfusions for refractory sideroblastic anaemia develops progressive arthralgias. Findings on hand radiographs are suggestive of calcium pyrophosphate deposition. Iron studies are obtained, showing a transferrin saturation of 92% and serum ferritin of nearly 2600 picomols/L.
