A patient is brought into A&E by his sister after she came to visit and noticed that he told her that the Queen had given him a specific task of building her a motorbike and that he is the only person in the country who can do it right. She says his fridge was empty and that she didn’t think he had been eating or sleeping. 

When trying to get more information from the patient, he talks continuously about his motorbike and other business ventures he has started up.
