An 18year-old male visits his GP after experiencing sudden onset, severe left scrotal pain

An 18year-old male visits his GP after experiencing sudden onset, severe left scrotal pain which lasted for 1 hour yesterday. He has experienced 3 similar episodes over the past 2 months. He had not sought help before, as the pain always spontaneously resolved within 1-2 hours in the previous episodes, but is now concerned that it keeps happening. He describes scrotal redness and swelling associated with the pain, and sometimes feel nauseous because of it. He is otherwise completely well and has no other symptoms in between the attacks. Today there are no remarkable features on examination, aside from an abnormal lie of the left testicle.
