A 64-year-old man presents to his GP with a 1-month history of a progressive dyspnoea

A 64-year-old man presents to his GP with a 1-month history of a progressive dyspnoea, unintentional weight loss and haemoptysis. He has recently been suffering from polyuria, polydipsia, muscle weakness and bony pain for the past 3 days. He suffers from COPD and has a smoking history of 1 pack a day for 35 years. On examination, you notice finger clubbing and generalised weakness across all four limbs. You request a chest X-ray. Blood results come back as normal except for hypercalcaemia. Work-up of hypercalcaemia reveals a raised parathyroid hormone-related protein (PTHrP) level.
