A 77-year old gentleman complains to his GP of a worsening 3-week history of breathlessness, dry (non-productive) cough and fatigue. Examination reveals decreased breath sounds, dullness to percussion on his left side and a current weight of 45kg (down 8kg in the past month). He is a retired non-smoker with an extensive occupational history in roofing and insulation.
