A left-handed 63 year old male is brought into A&E via ambulance. On examination he is:
- Alert, unable to provide verbal responses (dysphasia)
- Appears to have unilateral weakness localised to the entire left side of the body (face, arm and leg)
- Left homonymous hemianopia
- GCS = E:4, V:2, M: right side = 6, left side = 1
After taking a collateral history it was found that his symptoms suddenly occurred while he was home. This was witnessed by his daughter who called the ambulance. Past medical history includes hypercholesterolaemia and type 2 diabetes mellitus (diagnosed 11 years ago). A head CT shows an ischaemic stroke.
