A 67-year-old male presents to ED with shortness of breath and chest tightness. His saturations are 82% on air and he is started on high flow oxygen. His ECG shows sinus tachycardia only and his troponin is not elevated.
His bloods show the following:
| Hb | 71 g/L | (130-180 g/L) |
| WCC | 112 x 10⁹/L | (3.6-11 x 10⁹/L) |
| Neutrophils | 0.1 x 10⁹/L | (1.8-7.5 x 10⁹/L) |
| Platelets | 48 x 10⁹/L | (150-400 x 10⁹/L) |
| Film comment | Confirmed anaemia and thrombocytopenia. Blasts present with Auer rods | |
A chest x-ray is performed which shows bilateral pulmonary oedema. The haematology registrar calls and reports that flow cytometry is being performed but the appearances are suggestive of AML.
