A 67-year-old woman presents with a 4-month history of an ulcer on the medial aspect of the left leg. She had a previous deep vein thrombosis in the same leg and has chronic leg swelling. On examination, her legs are oedematous, but the left is more so than the right. There is a large shallow ulcer with surrounding hyperpigmentation of the skin. She feels well in herself and has not had any fevers.
