You see an 82-year-old on the medical take who is complaining of worsening shortness of breath over several months. You examine her and note that there are fine bilateral crackles on her chest. She also has a tremor, worse in her right hand than her left, an unsteady gait, masked facies and slow speech. Her past medical history includes Parkinson’s disease (PD) and hypothyroidism. She usually lives with her husband, who is her main carer. He manages all of her medications and is on his way in. The patient is not sure what her regular medications are but knows she takes several for her PD.
