A 20-year-old backpacker travelling through India presents to a local hospital with profuse cloudy diarrhoea. She reports opening her bowels 15 times a day but denies the presence of blood. She was initially managing with oral rehydration solution but has been feeling increasingly weak, and following an episode of loss of consciousness, decided to seek medical advice. Microscopy and Gram stain show large quantities of Gram negative comma-shaped bacteria.
