A surgeon presents with nocturnal perianal itching and discomfort. He recently returned from 3 months working in a Doctors Without Borders initiative in rural areas of South America, where he was living in large, shared facilities. His wife has begun experiencing similar symptoms but did not travel with him. On examination, you observe erythema and excoriations around the anus. A bedside adhesive test tape is positive.
