A 24-year-old man has a sudden onset of hearing voices narrating his actions. He believes his neighbours are spying on him and appears suspicious. On mental state examination, tangentiality and clanging are noted. After two weeks, the patient’s symptoms resolved completely. PMH includes anxiety three years ago. He has no previous similar episodes.
