A 71-year-old woman with liver metastases has significant right upper quadrant pain

A 71-year-old woman with liver metastases has significant right upper quadrant pain. She currently takes 10mg morphine modified-release (MST) BD and has 2.5mg Oramorph PRN, usually around 4 times a day. Her pain has not improved since commencing morphine, and she is now complaining of pain in her right shoulder. She would like to discuss alternative analgesia. What would be the next best step in the management of her pain?
