A 72-year-old man with prostate cancer suffers with bone pain from metastatic disease

A 72-year-old man with prostate cancer suffers with bone pain from metastatic disease. His pain level is usually 2/10, which he is content with, taking 15mg morphine MR twice daily. He takes 5mg immediate-release morphine as required. His pain has been worse over the last few weeks and is now at a constant 7/10. Taking his breakthrough analgesia three times daily brings this down to 4/10. He has come to the clinic to try to improve his pain control.

How would you change his treatment?
