Blood tests
- Full blood count- look for rise in white cells in infection, anaemia in autoimmune conditions such as rheumatoid arthritis, and platelet count-this is important if considering joint aspiration.
- Urea and electrolytes- deranged renal function can impact treatment choice- many drugs are renally excreted for dose adjustments and need to be considered.
- Urate- can be raised in gout but can be normal in acute flares
- CRP- raised in inflammatory conditions and infection
- Lipids profile and glucose- raised in metabolic conditions such as diabetes mellitus and hypercholesterolaemia. Elevated serum insulin levels affect the renal excretion of uric acid, therefore increasing urate concentration in the blood. Some studies demonstrated an association between high plasma triglycerides and elevated urate levels. This is thought to be related to triglyceride synthesis and de novo synthesis of uric acid by the liver.
