A 40-year-old female presents to the acute medical unit with shortness of breath

A 40-year-old female presents to the acute medical unit with shortness of breath,  pleuritic chest pain and haemoptysis. D-dimer was raised, and a CTPA revealed a filling defect within the left pulmonary artery. Her medical history included three miscarriages at 6-9 weeks gestation, but no cause had been identified. She also had a previous unprovoked DVT 9 months before her admission, which was treated with apixaban. Examination revealed bilateral livido reticularis along her lower limb. She was hypertensive with 2+ protein in her urine. Blood tests showed thrombocytopenia.
