A 65-year-old presents to your clinic with central chest pain, sweating, shaking and nausea.

A 65-year-old presents to your clinic with central chest pain, sweating, shaking and nausea. He has had several episodes like this in the last few months. His ECGs and blood tests are always normal. He is a diabetic and has hypertension. He has undergone a cardiac stress test which has also come back negative. He has been using a GTN spray which doesn’t help his symptoms. Whilst talking to him, he reveals that his wife died of cancer a few months ago and that these symptoms start shortly after looking at pictures or after remembering something about her.
