A 51-year-old female presents with progressive shortness of breath.

A 51-year-old female presents with progressive shortness of breath. Her breathlessness is exacerbated by lying flat. She has no other medical history other than episodes of involuntary movements of the right leg for several months, for which no cause has been found. On examination she is afebrile and has no peripheral stigmata of disease. There are bi-basal crepitations, and new systolic murmur heard loudest over the apex. An echocardiogram demonstrates a dilated left atrium with a mobile mass in the vicinity of the mitral valve. What is the most appropriate management of the presentation described?
