An 80-year-old man is seen at a routine clinic. He has had Parkinson’s for 17 years and is on Levodopa/Carbidopa (25mg/100mg) every 4 hours. He lives in sheltered housing and is mobile with a zimmer frame. He complains that his symptoms are worse in the hour prior to taking his dopamine, and he tends to have difficulty walking. His family also report an increasing tendency to hallucinate people in his room, which he can find distressing, as well as a deterioration in short term memory and occasional confusion. What is the most appropriate treatment for some of these issues?
