A 33-year-old female with known CKD with proteinuria secondary to IgA nephropathy attends an outpatient acute ambulatory care clinic for a follow up chest x-ray after pneumonia 6 weeks ago. She takes ramipril 10mg once daily but held this during her pneumonia as per sick day rules and reintroduced this 4 weeks ago. She now feels very well in herself. In clinic, her blood pressure is 113/81. Blood tests performed in clinic show a creatinine of 145µmol/l (baseline) and a uPCR of 156 mg/mmol. She states she wants to try for a baby.
