A 14-year-old boy whose family has recently moved from Nigeria is brought into A&E with significant pain in his knees, legs and abdomen. He has had a mild cough and a low-grade fever over the last few days.

His parents report similar episodes whilst he was younger which always settled with paracetamol, and his older sister had similar presentations and started on some treatment back in Nigeria. Both of his parents have no known medical conditions.
