A 45-year-old known cirrhotic presents to the emergency department. He has come in with dehydration secondary to diuretics for ascites and has abnormal renal function tests. His baseline creatinine is 45 µmol/L, however this has risen to 150 µmol/L. Urine dip and renal ultrasound are both normal.
Diuretics are suspended and IV sodium chloride is given. However, creatinine does not improve.
