An 80-year-old male is reviewed on a ward round in a hospice. He has a diagnosis of metastatic prostate cancer and decompensated liver cirrhosis. His pain remains stable however routine bloods have shown a deterioration in renal function. He does not wish to be transferred to hospital. He usually takes morphine modified release 10mg PO twice a day with breakthrough doses. His eGFR is 28ml/min. Which opioid would be most appropriate given the potential toxicity?
