A 40-year-old male with ulcerative colitis and T2DM, normally managed with metformin and dapagliflozin, is admitted to the acute medical ward due to rectal bleeding and profuse diarrhoea. Bloods reveal an acute kidney injury with a Cr of 161 µmol/l (normal 55-120) and a GFR of 28 mL/min/1.73² (normal >90) and has been started on high-dose hydrocortisone for a colitis flare. He is currently struggling to eat and drink, and his blood glucose level is 17 mmol/l. What would be the best approach for managing his diabetes?
