A 35-year-old female of South Korean origin presents to rheumatology clinic. She has been feeling generally unwell for a few months with fatigue and unexplained weight loss. She has also had some generalised muscle and joint pains, which had been going on for a similar length of time. She also experiences intermittent pain in her left arm, which is worse with exercise.
On examination, she had a blood pressure of 88/70 in the left arm with an impalpable radial artery and weak brachial. She has a palpable radial pulse in the right arm with a blood pressure of 115/75. Carotid pulses are palpable bilaterally, although she complains of tenderness on the right side on palpation.
Blood tests are unremarkable.
