You see an 85-year-old gentleman with your GP tutor during a home visit. This gentleman has been bedridden for two years due to a stroke. Both of his arms and legs are paralysed, and he is unable to mobilise independently. He was diagnosed with Alzheimer’s dementia one year ago, and his cognitive function has been declining gradually. He has a background of diabetes. His wife, who is his primary carer is very concerned over his lower back’s wound. On examination, you see the picture below. What is the most likely diagnosis?
