A 40-year-old female presented with increased fatigue and feeling sleepy over the last few months.

A 40-year-old female presented with increased fatigue and feeling sleepy over the last few months. She also reports weight gain, even though her diet is still the same. She also noted that her hair is becoming thinner. Her periods are irregular but heavy. She is concerned about menopause. On examination, she has a HR of 50 bpm with cold and dry peripheries. She has a painless goitre with a rubbery consistency and symmetrical enlargement. Her past medical history includes T1DM, and she is on regular insulin. Her TSH is 7 mU/L and free T4 is 2pmol/L.
