A 54-year-old female presents with a rash on her shoulders, ankles and abdomen

A 54-year-old female presents with a rash on her shoulders, ankles and abdomen. She appears very distressed and says it is completely intolerable. On examination there is evidence of a blistering rash with erythema and bleeding from excoriations. The patient has lost around 8kg in the last year, and complains of smelly stools and intermittent abdominal cramping.
