A 33-year-old man complains of having very itchy bumps on the posterior aspect of his right calf. Around 8 months ago he had a mosquito bite in the same area that became intensely itchy. The itching has worsened and sometimes disrupts his sleep, and his repetitive itch-scratch cycle has caused more bumps to appear. On examination the bumps are around 1cm in diameter and are firm to touch. Some of the bumps are crusted over where trauma from scratching has caused bleeding. The patient is apyrexial and has mild asthma, which is well controlled with a salbutamol inhaler.
