A 36-year-old man presents to primary care concerned about some pink-purple skin changes on his legs

A 36-year-old man presents to primary care concerned about some pink-purple skin changes on his legs. He denies any bleeding, pain or itching. His past medical history is notable for previous intravenous drug use – although he says he has “been clean” for over 6 months. On examination, there are multiple multifocal, asymmetric, pink-purple nodular lesions over the skin of both calves and shins. On closer questioning, he reports some weight loss and fatigue.
