A 55-year-old patient presents to A&E complaining of dizziness and generally feeling unwell. They were recently diagnosed with giant cell arteritis and started on high dose prednisolone. They say they initially felt much better, but progressively got worse. On examination, his hands are puffy and you note telangiectases on the hands. On further questioning, he mentioned his fingertips have started changing colour when he forgets to wear his gloves. Vitals indicate moderate hypertension and investigations show anaemia with significantly deranged renal function. ECG shows sinus rhythm.
