A 76-year-old woman presents with itchy, red, swollen legs

A 76-year-old woman presents with itchy, red, swollen legs. She describes having noticed these changes some months back, but in the past few weeks then they have become worse.

Her legs are dry and a dark red in colour. The skin feels tender and tight over her shins, and when palpated feels thickened. She has a past medical history of eczema, hypertension, and ischaemic heart disease.
