A 32-year-old man presents to his GP with a two-week history of a dull ache in his scrotum. The pain has come on gradually in the left side of his scrotum, and there is swelling. He has also noticed that it feels an ‘odd’ consistency when he checks in the shower. On examination, there are distended blood vessels over the left side of the scrotum which are palpable.
