A 40-year-old man who is a builder presents to ED with an acutely painful scrotum

A 40-year-old man who is a builder presents to ED with an acutely painful scrotum. The pain started when he got out of bed this morning. The pain is worse when he coughs. He noticed a lump in his scrotum a few days ago. On examination, the right side of his scrotum appears erythematous and slightly swollen. There is a tender, palpable mass superior to the right testis, but you cannot get above this on palpation. He is afebrile and has normal observations.
