An 80-year-old lady who is normally fit and well is admitted to a general medical ward due to severe dehydration secondary to viral gastroenteritis. You begin IV rehydration with Hartmann’s solution and administer IV metoclopramide. Shortly after, you are called to see this patient due to a sustained lateral and upward deviation of the eyes bilaterally. The patient is unable to reposition her eyes and is very distressed.
