A 23-year-old man has been brought to the GP by his mother. She is very concerned about his behaviour, as during the past 6 months his personality seems to have changed entirely. He spends all of his time in his bedroom on his computer and stays up into the early hours of the morning. He tells you that he cannot sleep because he knows that the government has been monitoring his work and communications online, and if he sleeps then they will be able to arrest him. He says he knows this is the case because he has heard the secret agents having conversations about their plans.
