An 11-year-old boy is seen in A&E with a two-day history of mild unilateral scrotal pain. On examination, the scrotum is normal in appearance but has a small, firm and tender mass at the superior pole of the testis that shows a blue dot sign. The cremasteric reflex is present, and he is otherwise well. An US doppler confirms the diagnosis of torted hydatid of Morgagni.
