A 9-year-old boy is brought to his GP after experiencing 3 days of mild pain in his left scrotum. His observations are within normal parameters, and he has no past medical history. On examination, the scrotum is normal in appearance. On palpation, there is a small, tender mass in the superior aspect of the testis that is blue dot sign positive. The cremasteric reflex is intact, and the Phren’s sign is positive. The GP makes the clinical diagnosis of a torted testicular appendage.
