A 77-year-old man presents to his GP with a painless scrotal swelling

A 77-year-old man presents to his GP with a painless scrotal swelling. He explains that he noticed the swelling a couple of weeks ago, that there are no associated symptoms and that he feels well in himself. He has no past medical history and takes no regular medication.  

On examination there is a non-tender soft scrotal mass, the testes are palpable, and the mass transilluminates.
