What is a hydrocele?

A 55-year-old man presents with a painless swelling of his right scrotum, which has been gradually increasing over the past year. He denies any trauma or infection in the area. On examination, the swelling is smooth, and transillumination test is positive. There is no tenderness on palpation, and the testis is palpable separately from the swelling. The patient’s medical history is unremarkable. His routine blood tests are within normal limits.

Investigations: Ultrasound of the scrotum: Anechoic fluid collection around the right testis, with no solid components or septations. The testis appears normal.
