A 69-year-old retired banker presents to his GP with a 2-month history of a persistent cough, dyspnoea, unintentional weight loss and haemoptysis. He has recently been suffering from polyuria, polydipsia, constipation and nausea for the past week. He has a past medical history of COPD, a smoking history of 1 pack a day for 40 years but stopped 3 years ago. He has been seeing the physiotherapist for shoulder pain without much relief. As you put the oxygen saturation monitor on his finger, you notice clubbing. You request a chest X-ray. Blood results come back as normal except for hypercalcaemia.
