An 82-year-old lady is brought to see you with her son as her family are concerned about her. Her son reports that over the last 1-2 months she has become increasingly muddled. She has been struggling with everyday activities such as washing, dressing, cooking etc., and has become increasingly forgetful. She has no history of falls. She is not oriented to time, place or person and is unable to follow commands, so you find it very difficult to examine her neurologically. On observation as they leave the room, you notice she struggles to initiate movement and when she does, it is with a shuffling gait.
