A 78-year-old female on the respiratory ward for treatment of pneumonia

A 78-year-old female on the respiratory ward for treatment of pneumonia develops haematuria and bleeding from her oral mucosa. On examination, she appears unwell with HR 102, BP 88/50, and oxygen saturation 93% on 4L of oxygen. Blood tests reveal a platelet count of 18 (normal range 150-400×10⁹/L) and prolonged PT and aPTT.
