A woman with a history of Ankylosing Spondylitis presents to A+E with a 3 month history of lower back pain and reduced sensation “down below”. She has decided to come in today as she noticed faecal incontinence. Upon examination there is reduced sensation in the perianal area, with a slightly distended bladder. On PR examination anal tone is dramatically reduced.
