An 82-year-old woman, who is being treated for an infective exacerbation of COPD

An 82-year-old woman, who is being treated for an infective exacerbation of COPD on a geriatrics ward, has been noted to be increasingly fatigued. She has been refusing care and mobilising with the physiotherapy team, and declining meals. This is a marked change from her usual jovial self. On assessment, she is reluctant to engage with questioning before becoming tearful and saying that she has “no idea” where she is or what is happening and cannot find her husband (who died four years ago). She is suspected to be suffering from hypoactive delirium.

Which THREE of the following features would suggest a diagnosis of hypoactive delirium?  Select THREE options only.
