A 15-year-old girl is seen by a psychiatrist. She has been referred by her GP following parental concerns about her recent weight loss and is suspected to be suffering from anorexia nervosa. Physically she is visibly thin and pale, and her BMI is measured at 16.2, adjusted for age. She is reluctant to answer questions but explains how she “cannot eat, or I will get fat”. Her parents report that she avoids meals and weighs herself every day – and that this behaviour has been happening for around four months.
