A 24-year-old woman is reviewed by a psychiatrist, having been referred by her GP. She reports a three-month history of problems with her mood, describing having episodes where she feels incredibly low and then “on top of the world”. She says she seems to move between these two states frequently, generally every couple of days and seems to have no control over them. When she is “feeling high” she says she has lots of energy and feels restless, staying up into the early hours of the morning exercising. She has no relevant past medical history. Her uncle “was a manic depressive”.
