A 45-year-old man with a history of chronic alcoholism is brought to the hospital by his family. He has been increasingly confused and disoriented for the past week. He is experiencing difficulty with balance and coordination, and his family notes that his eyes seem to be moving back and forth rapidly. He is also complaining of double vision. On examination, the patient is disoriented to time and place, displays horizontal nystagmus, and has a broad-based, unsteady gait. There is no history of head injury or recent illness.
