You see a 19-year-old woman in the cardiology clinic who recently moved to the UK. She presents with multiple syncopal episodes. She tends to get very nervous in public and that’s when most of her episodes have come on. These episodes had been investigated abroad and she was subsequently started on anti-epileptic medication, however this has not yielded any benefit. The resting ECG and echocardiogram are both unremarkable. You organise an ILR to monitor her rhythm. On further assessment, you notice she has self-limiting episodes of VT with alternating QRS on a beat-by-beat basis. What is the likely diagnosis?
